Hi - member contacted us re the bolt problem. Would be situation where bolt was not fully tight. Constant fore / aft (brake and acceleration) would have eventually caused situation. Attached instruction sheet - alignment specs do highlight need to ensure bolts fully tight. Then use of the specially designed fail safe lock system to retain in place.
