If I took off the 20s and put on the 19s, would it be a simple install? Would spacers be required? Would it look odd with the fender flares? Wasn't sure if the current wide wheels in back would create an issue with less wide 19" tires and wheels.

I have 19" with m package, they are wider in the back, no need to reduce flares. Does not look as baddass as with 20" that are wider but is ok. |
The 19's in the US, unfortunately, are a square set up. |

Yup, I have them. My understanding is that it is regional. No idea why, makes no sense. Ours has 255's all around. Rear IMO should be at least 275's with those fender flares, if not wider.
