Depends what option you use, E-sys or Bimmercode. Bimmercode recomends coding with the engine shut off, E-sys, it is recommended to use it with the engine running (to avoid running out of electrical power). There is also Carly but i have not tried it and do not know for sure how it works.

No, ignition must be on. I also coded with the engine running, so it can be done. It will throw some errors that automatically clear up once you restart the car. Once its done and the car is restarted, everything should be ok (might need to setup clock again).

Just lube the steering column, I used a silicone based spray and it's quiet and works fine. Split the plastic column around the steering wheel and on the left side there is some gears and elbows, just spray them good. Worked for me, YMMV.
