Yes, the steering wheel is all the way "out". From what i can tell, my steering wheel is adjusted exactly as yours. I usually keep my left elbow on the top of the door too. I guess i will have to play with the top of the back seat some more. It is not a problem at all for anything under 1 hour drive. A 3.5 hour drive, i would probably start feeling the burning in my upper back, as if i have been shrugging for all that time.

This was a HUGE issue for me. My QX60 was so comfortable to drive. I do have the contour seats but had to put a lot of effort into adjusting the settings in order to make it work for me. I still wish the steering wheel would go a little lower though...

I have the same issue, the pain eventually went away after I purchased a memory foam lumber support pillow and started using it in the car. This is my biggest complaint about BMW. A high end X5 that is literally a pain in the butt.
