Grinding noise from front brakes when coming to a halt

As per subject - brakes make a "grinding" noise when coming to a halt from moderate speeds with moderate pedal pressure (very soft and very hard don't cause it).

Car has been to BMW twice, they remove the wheels, clean the brakes, apply paste, this gets rid of the noise for a week or two and then it's back.

What is the likely cause, and the permanent fix?

Were the brakes recently serviced? Sounds like either the pads or the rotors are getting glazed up, or maybe both. Which would suggest that there's a compatibility issue. Has the dealer told you what the root cause is? In other words, are the pads getting glazed...or the rotors...or both? Or, are the pads getting contaminated somehow (seems unlikely, but worth mentioning)?

Dealer has mentioned "contamination", and the noise does go away when they clean the brakes and apply paste. But then it comes back after a week/two, which indicates that it's not just contamination.
